The purpose of this letter is to inform you that your Providers will be transferring from University Family Healthcare P.A to Millennium Physicians Group effective December 8, 2025. The office will remain in the same location: 6731 Professional Pkwy W # 100, Lakewood Ranch, FL 34240. If you have an upcoming appointment with your provider, there is nothing you need to do. Please keep your appointment. Your appointment and medical records will already be available with Millennium Physician Group.

You will see the same staff at the same location and receive the same excellent care from your provider that you have come to expect.
